NOTES
Examination entries are only accepted if listed on
Examination Entry form 3.
All Student Membership numbers must be shown on Form 3.
No candidate will be accepted without a student membership
number.
Centres sending Membership Application forms to ACP by
the dates shown below will receive their membership numbers in
time for completion of Entry form 3.
Entry forms not posted by the dates shown below will be
classed as 'Late Entries'. If your entry form is for Late Entries
you must;
Telephone ACP your centre for a late entry reference
number
Enter this reference number on Form 3 in the column
provided
Enclose the Late Entry fee of £5 per subject for each
student entered.
